Hello! The most interesting heart problems I have worked with involved people who are born with heart problems. The heart can react very badly when stressed in a person who has been born with an inherited condition and once I was working with a patient who spontaneously development a very dangerous heart beat whilst sitting in front of me. She did not collapse or have any symptoms but as I had her monitored I was very concerned that something might happen to her. I also have worked with some amazing babies who have very bad heart problems but have recovered after surgery on their heart to correct the problem.
In answer to your second questions, some of my scientist colleagues answered a questions about death on Monday in this zone and essentially in death all organs and cells of the body gradually die and decay. The heart is no different.
